CCWM Patient Information
We welcome new patients at Comprehensive Center for Women’s Medicine. We are located at 100 E. Walton Street, Suite 400W, Chicago, Illinois 60611. Patient Information Note – there will be a charge of $50 if cancellations are not received within 24 hours prior to an appointment.
Our goal at Comprehensive Center for Women’s Medicine (CCWM) is to provide quality medical care in a timely manner. In order to do so, we have implemented an appointment/cancellation policy. This policy enables us to better utilize available appointments for our patients in need of medical care. All new patients are required to provide a credit card number when making an appointment.
We understand that seeing a doctor is only part of your medical experience. Billing and payment for health care services can be confusing and complicated. We try to help patients with insurance questions before their initial visit, so they can make an informed decision regarding their out-of-pocket costs.
Please call our office at 773-435-1150 prior to your visit to confirm that your medical visit will be covered by your health insurance provider.
Cancellation of an Appointment
If you cannot keep an appointment, we ask that you call us and give us at least 24 hours notice. This time will be reallocated to a patient who is in urgent need of treatment. Appointments are in high demand, and your early cancellation will give another patient the possibility to have access to timely medical care. Your assistance in avoiding appointment no-shows is appreciated. We will charge a fee of $50 if cancellations are not made at least 24 hours prior to an appointment.
How to Cancel Your Appointment
To cancel appointments, please call 773-435-1150. If you do not reach the receptionist you may leave a detailed message on the voice mail. If you would like to reschedule your appointment, please be sure to leave us your phone number and let us know the best time to return your call.
Late cancellations will be considered as a “no-show”.
A “no-show” is someone who misses an appointment without cancelling it in an adequate manner. “No-shows” inconvenience those individuals who need access to medical care in a timely manner. Patients who do not show up for an appointment will be charged $50.
